Question:

Can you help me out with what exactly we need for bringing our puppy over to Canada. I understand he needs his rabies shot - and vet paper work - I have heard that maybe we need our vet to sign off on some things also.

Answer:

Basically, according to the US State Department, all you need when bringing your dog into Canada is proof of rabies vaccination between 30 days and 3 years of the date of entry.
